Welcome to the Fingal Chamber Skillnet ESF Plus Programme Participant Form.
We are pleased to welcome you as a participant in our ESF+ programmes. Our programmes aim to help you and others foster an interest in lifelong learning and to develop skill.
We have developed this participant form to collect essential information about you so we can support you taking part in ESF+ Course. We need to do this so that we can comply with ESF+ regulations, and to make sure our initiatives are successful.
The information we collect about you includes details like:
- age,
- gender,
- education level,
- special category data, such as health information or ethnicity,
We will store this information securely and only for as long as necessary.
ESF+ stands for European Social Fund Plus. It is a European fund that focuses on lifelong learning through improving employment and education opportunities.
Your Rights and Choices
The form will ask for general and special category data about you. You can choose if you want to share the special category information in questions 13 to 18. If you do answer these questions, we will handle answers with utmost care.
You have the right to access your data, get us to correct it if we get your details wrong, and ask us to delete your data. You can also choose to opt out or withdraw at any time. However, if you withdraw, we will not be able to provide you with access to [ESF+ courses] until you reapply.

The first question in this form asks you to consent to your data being collected. At the end of this questionnaire, we will also ask you to declare that the information you have provided is true. If you would like to take part in the ESF+ course, you must answer all the questions even if they do not apply.
Questions 13 - 18 may not apply to you. If they do not apply to you, please select "I prefer not to answer".
Training Start Date
Education
National Framework of Qualifications (NFQ) in Ireland is a system that describes what learners need to know, understand, and be able to do to achieve a qualification. It lists the main qualifications awarded at each level and the pathways from one NFQ level to the next.
The NFQ has 10 levels.
- Level 1 and 2 awards cover basic literacy (reading, writing and spelling) and numeracy.
- Level 10 is a PhD (doctorate)

-EEA countries are EU Countries plus: Iceland, Liechtenstein and Norway.

Reasons for Collecting Personal Data
We use your personal data for the following purposes: 1) Delivering Skillnet Ireland training programmes via the Learning Network. 2) Monitoring the training programmes of the Learning Network. Skillnet Ireland is required by the DFHE to perform compliance reviews, evaluations of the training programmes, quality assurance and to verify attendance at training. You may be contacted either by telephone or email for these purposes. 3) For the purposes of providing aggregated statistical information to the DFHE to help inform national policy in relation to skills. You may be contacted by third parties, contracted by Skillnet Ireland to perform monitoring and evaluation activities for these specific purposes only.
